All symptom items employ an 11-point numerical rating scale (response anchors: no symptom to worst possible symptom), the activity limitations a 5-point response scale (responses ranging from ‘no difficulty’ to ‘unable to do’), and emotions and lifestyle items a 4-point response scale (responses ranging from ‘not at all’ to ‘very much’). All single item and scale scores from range 0 to 10, with higher scores indicating more greater impairment on the underlying construct.
