What licensing fees are associated with the PDQUALIF?
The PDQUALIF is available to academic researchers without fee. Pharmaceutical companies wishing to use the instrument will be charged $25.00 per patient enrolled in their study. Click here to request a license.
Can I modify the questionnaire wording?
The questionnaire cannot be called the PDQUALIF if questions or responses are changed or questions are added or deleted.
What methods of administration are available for the PDQUALIF?
The PDQUALIF was originally developed in Us English and validated for paper administration. Neither the original U.S. English nor any of the translations have been adapted for tablet, PDA, web or IVR administration.
Can I reformat the questionnaire?
The PDQUALIF may be reformatted (e.g., different font, boxes or blanks) to make it consistent with other instruments used in a study.
